hi my name is sippy landau and this is a
tutorial brought to you by amudim on
filling out your pre-flight travel entry
declaration form
the first thing we're going to do is
we're going to go to amudim.org forward
slash
traveler entry declaration form
or you can go to the mujim homepage at
amudem.org click on the travel to israel
button
scroll down and find the pre-flight
travel entry declaration page once on
this page you're going to scroll past
our updates to the
pre-traveler entry declaration form and
click on click here to get form
this will open the declaration form that
we need to fill out by scrolling down to
the bottom of this page
and clicking ok let's start
the next page is going to ask us how
we're arriving in israel select by ear
and then confirm and continue
the next step is to fill out our
personal information we're going to
start with name you have to fill out
your name spelled the exact same way
that it's going to be in the passport
that you're traveling with so we're
going to put our first name in and then
our last name
see a question that says are you a
citizen permanent or temporary resident
of israel this is automatically toggled
and if you're an israeli citizen or a
toshiba keep it as yes if you're not an
israeli citizen click the switch and
make it say no for citizenship we're
going to put in the country that we have
our passport issued from so i'm going to
put in united states over here and for
the passport number you're going to
enter the passport number of the foreign
passport that you're going to be using
to travel
the next thing we're going to be filling
out is our date of birth
remember that because we are filling
this out on an israeli website we're
going to do the day and then the month
and then the year so january 15 1980 is
going to be 1501
1980.
next we need to put in our phone number
we're going to start with the country
code
so for the united states that would be
plus one you can either type the
you can type out the country code that
you're coming from in letters and then
select the correct one or you can type
in
plus one
for united states and then that'll come
up as an option as well
when you're entering the phone number
don't use any special characters no
dashes just put in the digits of your
phone number
and then the next option of putting in
your landline that's optional you don't
have to use that
next we're going to put our email
address in
and the name and phone number of an
emergency contact then you're going to
click confirm and continue on the bottom
of the screen
next page is itinerary the arrival in
israel is going to be filled with
bengorian
airport of entry
do not change that leave that as is
the next question of airline enter the
name in israel start typing i'm going to
use ll for this example
and when you're entering your flight
number this is not your confirmation
number it's going to be on your
itinerary that you received it's usually
two to four digits
and make sure that you're putting in the
one that says flight number and not
confirmation number or itinerary number
the next question of the country which
you departed to israel this is talking
about the original country that you left
from
not including a stopover so if you left
from the united states and stopped over
in spain you're still going to be
putting the united states in over here
for date of departure because this form
has to be done within 72 hours of
leaving
to the airport you only have three
options for date of departure and select
between one of those three
the date of arrival is the same deal you
have three options select between one of
those three
when putting in your arrival time in
israel remember that we're working with
military time over there so if you're
arriving at 1 pm we're going to put in
13 o'clock
the next question is a check box
regarding transit to israel so if you
have a stopover flight
if your stopover flight is more than 12
hours you are going to have to fill out
the form again if your stopover is less
than 12 hours you're just gonna simply
fill in where you're stopping over
our next question is have you visited
another place or country in the last 14
days check this box only if you've been
to another country not including your
stopover and not the country that you
came from we're not talking about states
we're talking specifically about country
over here
the next check box is asking you if
you're going to be in israel for less
than 48 hours so that's if you're
traveling for a funeral or a medical
emergency
you need to check that if not you just
click continue
the next page is the health declaration
form
there's three options on top
we have written out an exact guideline
of which one to select depending on
where you were vaccinated or recovered
please read through all of the
guidelines and rules that we have put on
there
so that you don't fill it out wrong and
you don't run into issues you can find
it on the travel page
under the section of page 5 health
declaration
once reading through
our guidelines
then select the option that's most
appropriate to you next you need to add
all the information regarding your
vaccination boosters and if you've
recovered from covid
put in the vaccination type that you
received
as well as the date that you received it
and remember to do it the israeli way so
the day first and then the month
next question that they're going to ask
is if you have a covet certificate
for more in detail information as what
this refers to
check out our website on vaccination and
recovery
for uploading your vaccination and or
recovery letter click on the upload icon
near upload covid certificate do not
next you're going to see this green
pop-up
you're going to click continue to fill
out form
if you click the cancel
option it's going to clear your entire
form and you're going to start over so
make sure to click continue to fill out
form
and the next important note is
to choose a file in the folder do not
try to drag and drop a file from your
desktop because that will also clear
your entire form
after you upload your file if you see
the pop-up message it says sorry unable
to verify the certificate
this is not an error you can click
continue anyway
this is something that most people will
see unless you have a specific qr code
continue scrolling and you're going to
see
their rules and regulations that you
have to agree to read through it
carefully
and then check the boxes that you have
read through it and that you agree to it
confirm and declare
that as well
and then click confirm and continue to
move on to the next page
our next page is about adding additional
travelers
if you have additional travelers to add
then you can add the travelers here
if not
then just click confirm and continue
by the time we reach here we're almost
at the end of the form we're going to
start by entering our isolation address
by locality start by entering the city
name
type slowly because the list is going to
pop up as you type
same with street
for house number enter the number of the
house that you're staying at and now
apartment number if you're staying in an
apartment complex put the number of the
apartment if you're staying in a private
home that doesn't have an apartment
number just put the number one in there
continue filling out the zip code the
number of household members how many
people are there
and the number of rooms of accommodation
the next question that you'll see is
their caregiver
is
is a caregiver required for any of the
people who must go into isolation
um if yes then you insert the
caregiver's name if not just leave it
out and skip it
and then select i have read and
understood the isolation requirements
now we're going to click confirm and
continue and that is our final step
and that's it you have now completed and
submitted the mandatory traveler entry
declaration form
you should receive an email within 15
minutes
with your clearance to board the flight
the email will come from no reply at
moh.gov
dot il
if you don't receive the email within 30
minutes check your spam
if after an hour you still have not
received the email you're going to have
to fill out the form again
attached in the email you should have
either one pdf attachment or two pdf
attachments
one is titled incoming statement
everyone receives that attachment and
the other one is a green pass that's
only for those who have been issued a
tavi roach or green pass proactively
please make sure to read up on our
section of what to bring to the airport
to make sure you have everything with
you
good luck and safe travels
